These are people who did not type the clinic's name. They typed what they needed — a skin doctor, a hair clinic — and Google showed Adityan on the first page.
The positions are for phones only, over the last three months. We leave desktop out on purpose: a large share of desktop “searches” come from ranking tools, not people, and they make the numbers look better than they are.
One honest gap: the broader search “dermatologist Madurai” sits at position 12.5 — page 2. That is the next one to win.

Some of the clinic's health guides rank very high across India. They get enormous numbers — and very few of those readers ever become patients.

The back-acne guide ranks around position 3 and was shown 211,388 times. The vitiligo pages were shown about 163,000 times. But only about 1 in 1,000 people, or fewer, clicked, and most live far from Madurai.
